Medicare SNPs are a type of Medicare Advantage Plan (like an HMO or PPO). Medicare SNPs limit membership to people with specific diseases  or chronic conditions such as Diabetes and or heart failure. Medicare SNPs tailor their benefits, provider choices, and drug formularies to best meet the specific needs of the groups they serve, often better than a general plan. These plans require the doctor to submit the condition authenticity/confirmation form to the Insurance Company.

  • Medicare SNPs often have specialists in the diseases and conditions affecting their members. However, some SNPs cover out of network services and some don’t.
  • All SNPs must provide Medicare drug coverage (Part D).
  • In most cases, SNPs may require you to have a primary care doctor and you must get a referral to see a specialist in SNPs. (with a few exceptions)
